Referring Physician Information

Goodall deeply appreciates the referral of your patient to our Pain Management Centers.  Our goal is to work with you in helping your patient obtain pain relief and increased function and to reduce dependency on the healthcare system.  We will do everything reasonable to ensure the lines of communication are kept open and clear regarding the treatment and progress of your patient.  Our preference is to assume responsibility for your patient’s total pain needs in order to better track response to various treatment modalities; however, we can share some responsibility at your request.

How to refer your patients to Goodall’s Pain Management Centers

If you would like to have your patient seen at one of Goodall’s Pain Management Centers, please have your staff call 207-490-7932 (Sanford) or 207-490-7860 (Kennebunk).  Patients referred to the Pain Center will be seen after relevant medical records and an insurance referral (optional) are received in our office.

Medical records should include related office notes, hospital admission notes, MRI/CT/X-Ray notes and pertinent lab work. If the patient has had recent and pertinent X-Rays or MRIs, please remind him/her to bring the film(s) or disk(s) to the initial appointment.

If there are other issues not apparent in the medical records that are relevant to the referral, please feel free to contact us to briefly discuss the issues.

Following your initial phone call, we will contact the patient to set up an appointment.  The patient will be sent a packet of information to be completed and brought to their initial appointment.

You may also print, fill out, and fax the following referral form to 207-490-7936 (Sanford) or 207-604-7409 (Kennebunk)
